Mechanical Ventilation: Physiology & Clinical Applications

چکیده
It is estimated that, by the year 2000, fifty percent of the population in the United States will be over age sixty. The "baby boomers" will have reached their "golden age." This situation will have far-reaching ramifications for society in general and will also cause the medical profession to evaluate critically its treatment of the elderly. In light of this, Dr. Sculco's text is a timely effort, and one that goes a long way toward addressing musculoskeletal disease and treatment in the elderly. The book is a collaborative effort of twenty-five authors, edited by Dr. Sculco. While it is certainly not meant to be an exhaustive reference, in its four hundred and eight pages it offers a broad overview of orthopedic treatment of the elderly and can certainly serve as a springboard to more detailed reading. The text is divided into four sections: the first dealing with the evaluation of the geriatric patient, the second with specific musculoskeletal dysfunction and treatment in the elderly, the third with medical diseases associated with musculoskeletal abnormalities in the aged (e.g., metabolic bone diseases), and the fourth with nursing care and rehabilitation of the elderly. Each section contains three to six chapters, all of which are well referenced. The book is intended for any physician who deals with elderly patients and will certainly be of much use to the orthopedist, gerontologist, internist, and family practitioner.

Clinical review: Respiratory mechanics in spontaneous and assisted ventilation
Pulmonary disease changes the physiology of the lungs, which manifests as changes in respiratory mechanics. Therefore, measurement of respiratory mechanics allows a clinician to monitor closely the course of pulmonary disease. Here we review the principles of respiratory mechanics and their clinical applications. These principles include compliance, elastance, resistance, impedance, flow, and w...
متن کاملThe Effect of Intravenous Tocilizumab Therapy on the Prognosis of Patients with Covid-19: A Case-control Study
Background and Objective: Tocilizumab is a monoclonal antibody against the Interleukin-6 (IL6) receptor, but its effect on improving the clinical condition of coronavirus disease of 2019 (COVID-19) patients is debated. Accordingly, this study investigated the impact of Tocilizumab as an alternative treatment on the outcomes of covid-19 patients. Methods: In this retrospective case-control stud...
متن کاملClinical management of pressure control ventilation: An algorithmic method of patient ventilatory management to address "forgotten but important variables".
Pressure controlled ventilation is a common mode of ventilation used to manage both adult and pediatric populations. However, there is very little evidence that distinguishes the efficacy of pressure controlled ventilation over that of volume controlled ventilation in the adult population. This gap in the literature may be due to the absence of a consistent and systematic algorithm for managing...
متن کاملMechanical ventilation: physiological and clinical applications
Mechanical Ventilation provides a comprehensive theoretical background and practical approach to mechanical ventilation. It is written primarily for respiratory therapists, but critical care and respiratory physicians and nurses will find many aspects of the book invaluable, particularly the clear and concise descriptions of the different modes of mechanical ventilation, their advantages and di...
متن کاملVariable mechanical ventilation
Objective: To review the literature on the use of variable mechanical ventilation and the main outcomes of this technique. Methods: Search, selection, and analysis of all original articles on variable ventilation, without restriction on the period of publication and language, available in the electronic databases LILACS, MEDLINE®, and PubMed, by searching the terms "variable ventilation" OR "...
متن کاملEsophageal and gastric pressure measurements.
The measurement of esophageal and gastric pressures with balloon-tipped catheters has been used with great success over the past half century to delineate the physiology of the mechanical respiratory system. Pleural pressure and abdominal pressure values estimated from esophageal and gastric pressure measurements allow analysis of lung and chest wall compliance, as well as work of breathing, re...
